The Direct Care Worker is responsible for teaching skills and facilitating Community Integration, behavioral support services, and crisis intervention services according to the individualized psychiatric rehabilitation plans. The Direct Care Worker plays a vital role in the Psychiatric Rehabilitation Program, facilitating the coordination and delivery of services to individuals with mental health challenges. This position is responsible for client assessment, service planning, and ensuring each individual receives tailored support to achieve their goals.

Essential Duties and Responsibilities:

  • Client Assessment and Service Planning:
    • Conduct comprehensive assessments to determine individual strengths, needs, and goals.
    • Develop individualized service plans in collaboration with clients and program staff.
    • Monitor progress and adjust service plans as needed.
  • Skill Development and Support:
    • Facilitate skill-building activities focused on daily living, socialization, and vocational skills.
    • Provide guidance and support to help clients develop and practice essential life skills.
    • Encourage independence and empower clients to take control of their recovery journey.
  • Community Integration:
    • Assist clients in identifying and accessing community resources, including educational, vocational, and recreational opportunities.
    • Foster connections with local organizations, support groups, and other community-based services.
  • Collaboration and Communication:
    • Work closely with program staff, therapists, and external stakeholders to ensure coordinated and holistic care.
    • Communicate regularly with clients, families, and support networks to provide updates on progress and address any concerns.
  • Documentation and Reporting:
    • Maintain accurate and up-to-date client records, including service plans, progress notes, and other documentation.
    • Prepare regular reports on client outcomes and program performance for program leadership.

Education & Experience Requirements:

  • Bachelor’s degree in social work, Psychology, Counseling, or related field.
  • DLA-20
  • Strong understanding of psychiatric rehabilitation principles and practices.

Our Rehabilitation Specialist position provides person-centered support in a team environment to consumers both on-site at the program and in the community. The focus is on teaching skills both individually and in groups and accessing resources for resiliency, recovery, independent living, and success in life. It includes assessing consumers’ needs and strengths, assisting with the development of the rehabilitation plan, and assessing progress. Also included is supervising the Direct Care Workers and ensuring they meet productivity measures and daily documentation requirements. Additionally, the Rehabilitation Specialist coordinates psychiatric rehabilitation services and develops community resources.

Essential Duties and Responsibilities:

  • Conduct intake/assessments/IRP development, IRP reviews, Face to Face screenings with client to include DLA-20 assessment and medical necessity criteria review.
  • Responsible for creating the Individual Rehabilitation Plan (IRP).
  • Provide orientation, supervision, and training to Direct Care to ensure support and goal attainment for participating clients.
  • Provide ongoing training and support to the Direct Care Worker, which is related to developing and implementing individual and group interventions tailored to the client’s specific needs of the client(s).
  • Educate the Direct Care Worker on how to provide rehabilitation services that correlate to the goals identified in the Individual Rehabilitation Plan and how to document such services appropriately.
  • Communicates regularly with the Direct Care Worker as needed to ensure clinical documentation is completed and any barriers regarding completion of duties are noted and addressed.
  • Meet with the Direct Care Worker weekly for supervision.
  • Enforce and comply with the agency’s electronic documentation entry and approval timelines.
  • Maintain accurate and current information on all client databases.
  • Monitor client satisfaction of services and ensure Direct Care Worker possess and utilize strong rehabilitative skills in delivery of services.
  • Document all planned and unplanned discharge summaries within 15 days of discharge date.
  • Provides referrals to other public and private community agencies upon discharge of client for mental health support.

Education & Experience Requirements:

  • Must have a Certified Psychiatric Rehabilitation Practitioner (CPRP)
  • DLA-20 Certification
  • At least 2 years of supervisory experience
  • 2 years as a Rehabilitation Specialist (preferred)
